Functional Capacity Assessment
An FCA report is a comprehensive assessment of a person’s functional ability across a range of daily activities and environments, to determine the supports needed to assist and achieve their goals. This report highlights the 6 functional NDIS domains, whilst commenting on a participants environment, living situation and provides input into the supports needed for the next NDIS plan period. These are often flagged by the NDIS to provide further supporting evidence when an NDIS plan has not been reviewed for a long period of time, to assist with advocacy when there have been large changes to the participants life.

AT LEAPS AND BOUNDS, THIS SERVICE ENTAILS THE FOLLOWING

A range of information gathering with the participant, their family and main supports

Functional observations within relevant environments (e.g. home, community, day-service, school)

Liaison with relevant stakeholders (e.g. support workers, therapists, school)

Assessments and assessment scoring based on functional areas

Report writing

Reviewing the report with the main caregiver to ensure that information is accurate

Submission of the report alongside supporting evidence
